PURPOSE: This report will be used to form the basis of an investigation into any accident or other incident which occurs before, during, or after a NJIBL sponsored event. This form shall be used to report any incidents including, but not limited to, any injuries, ejections, or poor behavior involving a player, coach, spectator, official, and/or umpire. In the case of injury, ejection, or incident involving threat of or actual physical violence, this report must be submitted to the Age Division Director by the Manager of the involved team within 24 hours of the incident. This form shall also be used by any concerned individual to report any incidents which the person believes should be addressed by the Board of Directors. Failure to timely submit the form may result in forfeiture of rights of an involved individual.
